CONSENT & RELEASE OF LIABILITY
As the legal parent or guardian, I release and hold harmless to the business, the entity doing business as Math Project, its owners, operators and employees, from any and all liability, claims, demands, and causes of action whatsoever, arising out of or related to any loss, damage, or injury, including death, that may be sustained by the participant and/or the undersigned, while in or upon the premises or any premises under the control and supervision of Math Project, its owners, operators and tutors or on route to or from any said premises.
Math Project reserves the right to terminate a student from the program at any time as deemed appropriate. All study materials and stationary provided by Math Project must be returned immediately in original condition upon leaving the program. No copies will be made or distributed of any study material provided by Math Project, unless permitted explicitly by Math Project.
The parents/guardians are responsible to accompany their children to and from the classroom every session. Parents will accept full responsibility for their child whenever they leave the classroom whether it’s before their session begins, during the session for washroom, water breaks, etc. or when it ends. Math Project does not assume responsibility for students who leave the classroom for any reason or arrive before classes begin or stay after the classes end.
I hereby consent to the videotaping/photographing/audio recording of myself or my child. I understand that all material obtained will be used by Math Project for educational and related purposes, including external distribution via satellite broadcast, cable TV, radio broadcast, webcast or on-demand streaming in digital format on the Internet. Math Project may also share this material with others, as appropriate. I understand that my submission (the Recordings) will become the property of Math Project and I understand that use of the recordings may include, but not necessarily be limited to, the following: Internal use at Math Project and other institutions in streaming digital format (live or on-demand) on the Internet or other digital media. Distribution may include digital reproduction, satellite broadcast, cable TV, webcast, on-demand or social media.
